If clinically appropriate, you receive a signed diagnostic certification from an MD, DO, NMD, or Ophthalmologist for submission to the MVD.
Submit to Arizona MVD
Mail your certified documentation to the Arizona Department of Transportation MVD Medical Review Program (P.O. Box 2100, Mail Drop 818Z, Phoenix, AZ).
