Arizona Requirements
Arizona Medical Window Tint Exemption
Arizona uses an official Motor Vehicle Division medical window-tint exemption process. Tint-RX provides physician review and, when medically appropriate, the documentation needed for the Arizona exemption process.
Arizona’s exemption process
Physician documentation supports your application; Arizona MVD makes the final exemption decision. Submission to Tint-RX starts a medical evaluation and does not guarantee documentation or an exemption.
- Complete the Arizona intake with your medical history and supporting records.
- A licensed physician reviews your medical need and may request additional information.
- If medically appropriate, receive documentation for the Application for Window Tint Medical Exemption (Form 40-1511).
- Submit the completed application to the Arizona MVD Medical Review Program using the instructions provided. Keep the required exemption certificate in each vehicle you own.
The exemption may apply to vehicles you own and other vehicles in which you are a regular passenger. Follow MVD’s approval and equipment requirements before changing your window tint.

Additional documentation for potential HSA reimbursement
Patients may request an additional Letter of Medical Necessity for medically necessary automotive window film. Tint-Rx offers this letter when clinically appropriate; it is not automatically included with an evaluation, and not every applicant qualifies.
- State-specific tint documentation
- Addresses the applicable state’s medical window-tint requirements.
- Letter of Medical Necessity for HSA purposes
- Explains why recommended window film is medically necessary for the individual patient.
Neither document substitutes for the other. HSA documentation does not authorize tint that violates applicable state law. A letter does not guarantee HSA eligibility or reimbursement, and no particular tint package is automatically eligible.
Please do not email medical records or payment information.
Medical certification supports your application for an ADOT/MVD window-tint exemption. MVD approval is required before relying on the exemption. The exemption does not authorize full-windshield tint. Explore Arizona tint laws.
